It is rare but there have been two crash’s involving DEF being mixed with Jet-A, I don’t remember the reason but the fueling trucks that fuel commercial aircraft carry bulk DEF and worse on those trucks it use’s the same connector as Jet A, if mixed with Jet A it will clog the fuel filters starving both the main engines as well as the Auxiliary Power Unit (APU).
As such even though the plane will taxi properly, but at take off under near full power having the fuel filters clog up will not produce good results.
